Why does AI get things wrong so often?
Usually because it is missing a constraint or a detail only you know, took a wrong assumption, or returned the wrong format, not because the model is weak. Naming which of those 4 happened points you straight at the fix.
Do I need to start a new chat when the AI misses?
No. Starting over loses the context and the parts it got right, and you tend to repeat the same omission. A recovery prompt in the same chat is faster and lands better. Restart only when nothing is worth keeping.
Which recovery prompt should I use first?
Read the symptom. Wrong direction means the reset; generic means ask-me-first; wrong look or tone means show-an-example; breaks a rule means add-the-constraint; off in a way you cannot name means make-it-critique-itself. Most misses need only 1.
How do I stop the AI from missing in the first place?
